Hours available 6am-6pm every Sunday and part-time from 6pm-6am every Wednesday. Pay rate $35.00 per hour.

Job Description

Our Primary Care team is looking for an exceptional nurse to help us manage and coordinate care for our high-acuity patients. Our nurses enjoy strong relationships with patients, collaborative work environments and a deep sense of belonging and appreciation. Nurses are responsible for carrying out a variety of healthcare services, including:

• Medication management;
• Coordinating care between outside providers and hospital systems;
• Disease management;
• Wound care;
• Recording vitals;
• Developing Plans of Care;
• Phlebotomy;
• Hydration Therapy and more!

Qualifications:

• Active Driver’s License
• Active LPN License
• BLS/CPR Certification
