Salary and Benefits
The salary range for this position is $270,000 - $300,000 per year, with opportunities to earn additional income based on performance. As a Chief Physician - Post-Discharge Care, you will be responsible for managing patients transitioning from hospital to home-based settings, ensuring seamless continuity of care. You will work closely with healthcare professionals, including PCPs, Advanced Practice Clinicians, Social Workers, Case Managers, and Pharmacists, to ensure optimal coordination of care.
Responsibilities
- Direct Patient Care: Manage patients transitioning from hospital to home-based settings, ensuring seamless continuity of care.
- Collaboration and Coordination: Work closely with healthcare professionals, including PCPs, Advanced Practice Clinicians, Social Workers, Case Managers, and Pharmacists, to ensure optimal coordination of care.
- Facilitate Interdisciplinary Meetings: Organize regular meetings to discuss patient care plans and address any concerns or challenges.
Requirements
- California Medical License: Valid and unrestricted license or eligibility.
- Clinical Experience: Minimum 3-5 years prior experience in Family Medicine or Internal Medicine.
- Driver's License: Valid California driver's license or ability to obtain one.
