Priority Care Nurse Practitioner - Primary Care - Transitional Care
Our client, an established and growing provider organization, needs an experienced Nurse Practitioner to provide post-discharge clinical care for a select patient population.
We are seeking a Nurse Practitioner for a Priority Care Program. In this role, you will join an innovative care delivery team located in the San Fernando Valley.
You will be an integral part of a health care delivery program designed to reduce readmission rates for patients with comorbid conditions.
Position Details :
- Provides comprehensive care in an outpatient clinic or community environment.
- Emphasis on post-discharge patient care leading to sustained stabilization
- Member of a supportive, collaborative Care Coordination team
- Ensures clinical guidelines are met to ensure quality of care
- Provides follow-up and referrals to social and behavioral health services
- Ensures the provision of high-quality, transitional care for more complex patients
- Facilitates interdisciplinary meetings for coordination of care
- Monday - Friday, 8 : 00 a.m. to 5 p.m.
