About the Role
We are seeking a physician to join an innovative care delivery team located in the San Fernando Valley. This role offers a unique opportunity for post-discharge clinical care for a select patient population.
Responsibilities
- Provide high-quality, transitional care for complex patients in an outpatient clinic/community environment
- Facilitate interdisciplinary meetings to ensure coordination of care
- Collaborate with designated PCPs and Nurse Practitioners
Requirements
- Board Eligible/Board Certified in Family Medicine
- Experience in Geriatrics or Emergency Medicine preferred but not required
- Active and unrestricted California Medical License, or eligible
- Minimum 5 years prior clinical experience required
- Expertise in both outpatient and hospital-based medicine a plus
