We are seeking an experienced physician to join our team at Gorecruitme as a key member of the Priority Care Team. This is a unique opportunity where you can make a positive impact on the continuum of care and drive change in patient outcomes.
As a director of post-discharge clinical care, you will work with a select patient population and collaborate with leadership in Population Health and Case Management to provide high-quality transitional care for more complex patients.
Responsibilities:
- Patient Care Direction: Directs patient care in outpatient clinic and/or home settings, providing high-quality care to patients transitioning from hospital to home.
- Care Coordination: Works closely with PCPs, Advanced Practice Clinicians, Social Workers, Case Managers, and Pharmacists as an integral member of the Care Coordination Team.
- Interdisciplinary Collaboration: Facilitates interdisciplinary meetings to ensure optimal coordination of care.
Schedule:
- Full Time, Monday - Friday Schedule
- No nights or weekends

Requirements:
- A valid and unrestricted California medical license
- Board Eligible/Board Certified in Family Medicine or Internal Medicine
- Minimum 3-5 years prior clinical experience
- A valid California driver's license, or ability to obtain
Compensation and Benefits:
- $270,000 - $300,000 Base Salary
