Responsibilities:
- Develop and implement effective care plans for complex patients
- Collaborate with PCPs and Nurse Practitioners to ensure seamless care coordination
- Facilitate interdisciplinary meetings to promote effective care delivery
- Work closely with a supportive Care Coordination team to achieve optimal patient outcomes
Qualifications:
- BE/BC in Internal Medicine
- Geriatrics or Emergency Medicine experience preferred but not required
- Active and unrestricted California Medical License, or eligible
- Minimum 5 years prior clinical experience required
