">We are seeking an experienced Physician to join our Priority Care Program. As a key member of our team, you will play a critical role in providing high-quality, transitional care for complex patients.
">About the Role
">This opportunity offers a unique chance to collaborate with designated Primary Care Physicians (PCPs) and Nurse Practitioners. You will ensure seamless coordination of care, facilitate interdisciplinary meetings, and provide post-discharge clinical care.
">Responsibilities
">• Provide care in an outpatient clinic/community environment
">• Facilitate interdisciplinary meetings to ensure coordination of care
">• Collaborate with a collaborative Care Coordination team
">• Anticipate a minimum of 4 days per week in the clinic setting
">Requirements
">• Board Eligible/Board Certified in Internal Medicine
">• Experience in Geriatrics or Emergency Medicine preferred but not required
">• Active and unrestricted California Medical License, or eligible
">• Minimum 5 years prior clinical experience required
