Job Overview
GoRecruitMe is seeking a highly skilled physician to join our innovative care delivery team in the San Fernando Valley. As a Priority Care Physician, you will be responsible for providing post-discharge clinical care to complex patients.
Responsibilities
- Provide high-quality transitional care to patients with multiple health needs
- Collaborate with designated Primary Care Physicians (PCPs) and Nurse Practitioners to ensure seamless care coordination
- Facilitate interdisciplinary meetings to discuss patient care plans
- Work closely with a collaborative Care Coordination team to support patient outcomes
Requirements
- Board Eligible/Board Certified in Family Medicine
- Experience in Geriatrics or Emergency Medicine preferred but not required
- Active and unrestricted California Medical License, or eligible
- Minimum 5 years prior clinical experience required
