We're looking for Residential Coordinator to join our team! As a Residential Coordinator, you will provide support and guidance to people served and their Shared Living Provider as they learn and develop important daily living skills and life activities to live as independently as possible.
SUMMARY OF MAIN RESPONSIBILITIES
Residential Coordinator manages Shared Living Providers AND DPS to ensure the highest quality services are provided to individuals in that setting. Maintain monthly, in person meetings at the SLP to ensure individual is well cared for. Manage a caseload of individuals that have developmental disabilities and mental health issues. Develop and maintain programs and attend biannual meetings.
SPECIFIC RESPONSIBILITIES :

- Ensure compliance with agency policies and procedures, contractual agreements established with Nebraskas Department of Health and Human Service System, as well as ensuring compliance with state, federal and local regulations and laws.
- Ensures client rights are maintained and respected via in-home / work visits, GER audits, and attendance at quarterly Human Right Committee meetings.
- Collaborates with Director of Operations (DO) and team to ensure client specific protocols, medications, or medical needs are in place and are adhered to by the Shared Living Provider and Day Staff.
- Collaborates with DO to ensure client safety plans, behavioral support plans, behavior trackers are person-centered, implemented, and required documentation completed.
- Quality Assurance of data collection and client records.
