This role focuses on providing mental health support, case management, and therapeutic interventions for individuals facing emotional and behavioral challenges. Job Duties:
- Conduct comprehensive biopsychosocial assessments to determine patient needs and formulate treatment plans.
- Provide short-term and long-term therapeutic interventions tailored to the healthcare setting.
- Coordinate patient care and discharge planning with hospital staff, outpatient providers, and community resources.
- Assist patients and families in navigating healthcare systems, including insurance, financial assistance, and social services.
- Facilitate support groups and psychoeducational workshops on mental health topics.
- Address social determinants of health that impact patient recovery, such as housing, employment, and access to care.
- Advocate for patient rights and ensure culturally competent, patient-centered care.
- Maintain detailed and confidential documentation in compliance with healthcare regulations and facility policies.
- Participate in interdisciplinary team meetings, case conferences, and professional development initiatives
Experience:
- 1-3 years experience with mental health patients
Education:
- Master's Degree (Required)
License:
- Active LCSW/LMFT issued in the state of ME
Qualifications:
- Ability and willingness to be credentialed with various insurance agencies
- Obtain NPI numbers
- Ability to create and implement functional treatment plans
