Responsibilities:
- Function autonomously to care for patients with complex, acute, critical and chronic health conditions.
- Prescribe medications including the frequency and dosage
- Work individually and in interdisciplinary teams of resources and consultants
- Facilitates patient transition within and between health care organizations to ensure accurate communication among the health care providers
- Record and maintain patient records in timely manner to ensure all services are accurately accounted for and available for review by the interdisciplinary teams
- Talk to patients about effectively managing their health, as well as assisting in designing appropriate treatment plans and recommending further care.
- Current Nurse Practitioner certification.
- Active state nursing license.
- Master's degree in nursing from an accredited program.
- A minimum of 3 years of clinical experience in a relevant specialty.
- Exceptional communication and interpersonal skills.
- Strong analytical and problem-solving abilities.
- Proficiency in both Spanish and English.
- Competitive salary range of $120,000-$130,000 per year, commensurate with experience and qualifications.
