Job Duties:
- Conduct comprehensive patient assessments, including medical history reviews and physical exams.
- Diagnose and treat acute and chronic conditions, prescribing medications as appropriate.
- Develop and implement individualized care plans with a focus on preventive healthcare.
- Perform minor procedures and administer treatments, such as wound care or injections.
- Order and interpret diagnostic tests, including lab work and imaging.
- Collaborate with physicians, specialists, and other healthcare team members.
- Educate patients and families on health management and lifestyle modifications.
- Maintain accurate and detailed patient records in compliance with HIPAA regulations.
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
- Valid and unrestricted New York State NP license
- Board certification as an FNP (Family Nurse Practitioner), AGNP (Adult-Gerontology Nurse Practitioner), or ANP (Adult Nurse Practitioner)
- Previous experience in primary care, family medicine, or a specialty field preferred
- Strong clinical assessment and diagnostic skills
- Excellent communication and patient education abilities
- Proficiency in electronic health records (EHR) systems
