Responsibilities:
- Conduct comprehensive health assessments, including physical exams and screenings, for a diverse patient population
- Diagnose and manage acute and chronic medical conditions, emphasizing preventive care and health promotion
- Develop and implement individualized treatment plans in collaboration with patients and multidisciplinary teams
- Provide culturally sensitive care and advocate for patients' rights and access to healthcare services
- Coordinate referrals and follow-up care to ensure continuity of care for patients
- Maintain accurate and up-to-date medical records in compliance with regulatory standards using electronic health record (EHR) systems
- Participate in community outreach events and health fairs to raise awareness and provide health education
- Stay current with internal medicine trends, evidence-based practices, and relevant healthcare policies
- Master's degree in Nursing from an accredited program.
- Current certification as a Nurse Practitioner.
- Active state Nursing license.
- Minimum of 3-5 years of clinical experience, preferably in community health or public health settings.
- Strong interpersonal and communication skills, with the ability to work effectively with diverse populations.
