About the Role:
We are seeking a dedicated and caring Patient Advocate and Support Specialist to join our team at Gotham Per-diem. In this critical role, you will provide essential support to our residents, empowering them to maintain their independence and quality of life.
Key Responsibilities:
- Resident Support: Offer emotional support and encouragement to residents, helping them to maintain a positive outlook and engage in meaningful activities.
- Medication Administration: Assist with medication administration, following established protocols and procedures to ensure accurate and timely delivery.
- Vital Signs: Take and record vital signs, including temperature, pulse, respiration, and blood pressure, monitoring for any changes or concerns.
- Care Documentation: Accurately document all interactions and interventions, maintaining clear and concise records of resident care.
- Interdisciplinary Collaboration: Work collaboratively with the interdisciplinary team to develop and implement individualized care plans, addressing the unique needs and preferences of each resident.
Requirements:
- Certification: Current certification as a Nursing Assistant or equivalent in the state of New York.
- Experience: At least 6 months of experience in long-term care, geriatrics, or a related field.
- Skills: Excellent communication, interpersonal, and problem-solving skills, with the ability to work independently and as part of a team.
