MDS Coordinator :
The MDS Coordinator is responsible for the coordination and management of the Minimum Data Set (MDS) process for skilled nursing facility (SNF) residents.
This includes working with nurses, therapists, and other healthcare professionals to ensure accurate and timely completion of MDS assessments, care plans, and other required documentation.
The MDS Coordinator also plays a key role in ensuring that the facility complies with all CMS MDS regulations.

MDS Coordinator Essential Duties and Responsibilities :
- Coordinate and manage the MDS process for all SNF residents
- Work with nurses, therapists, and other healthcare professionals to ensure accurate and timely completion of MDS assessments, care plans, and other required documentation
- Review and analyze MDS data to identify trends and patterns
- Develop and implement MDS-related quality improvement initiatives
- Ensure that the facility complies with all CMS MDS regulations
- Provide training and education to staff on MDS procedures and requirements
MDS Coordinator Qualifications :
- Registered Nurse (RN)(LVN)
- 4 years of experience in a skilled nursing facility setting
- 2 years of experience with the MDS process
- Strong knowledge of CMS MDS regulations
- Excellent organizational and communication skills
- Ability to work independently and as part of a team
MDS Coordinator Preferred Qualifications :
- MDS certification
- Experience with MDS software programs
- Experience with quality improvement initiatives
