About the Role :
A leading long-term care and rehabilitation provider is seeking a licensed Occupational Therapist (OT) to join our team in the San Francisco Bay Area . The OT will play a key role in helping residents regain independence and improve their quality of life through personalized therapy and rehabilitation programs in skilled nursing facilities (SNFs) and assisted living communities.
Key Responsibilities :
- Evaluate and develop individualized treatment plans for residents in skilled nursing and long-term care settings.
- Provide rehabilitative therapy to improve patients’ mobility, strength, and daily function.
- Collaborate with nurses, physical therapists, and speech therapists to implement holistic care plans.
- Educate patients and caregivers on adaptive techniques and equipment for home and facility use.
- Ensure compliance with Medicare, state, and federal regulations .
- Document patient progress and maintain accurate therapy records .
