Essential Duties
Patient Assessment and Care Coordination:
- Conduct comprehensive assessments of outpatient patients to identify clinical needs and develop individualized care plans.
- Coordinate with healthcare providers to ensure seamless delivery of care, including scheduling, follow-ups, and necessary interventions.
- Manage and optimize the utilization of clinical resources, including staff, equipment, and facilities, to ensure efficient and effective patient care.
- Monitor and evaluate patient care processes to identify areas for improvement and implement best practices in resource management.
- Maintain accurate and detailed documentation of patient assessments, care plans, and clinical outcomes in electronic health records (EHR).
- Prepare and submit reports related to patient care, resource utilization, and clinical performance as required.
- Work closely with outpatient care teams, including physicians, nurses, and other healthcare professionals, to discuss patient care plans and address any issues.
- Communicate effectively with patients and their families to provide education, address concerns, and ensure understanding of care plans.
- Ensure that all clinical activities adhere to hospital policies, regulatory requirements, and industry standards.
- Stay informed about changes in healthcare regulations and best practices to ensure ongoing compliance and optimal patient care.
Non-essential Duties
- Participate in departmental meetings and contribute to discussions on clinical resource management and process improvements.
- Assist with the training and orientation of new staff members on clinical resource management procedures and practices.
- Support administrative tasks related to outpatient care and clinical resource management as needed.
Preferred Experience:
- Minimum of 2 years clinical experience working with the behavioral health population preferred.
- Demonstrates familiarity with short-term crisis intervention, understanding of individual psychotherapy, group psychotherapy, and family intervention.
- Possess broad understanding of various theoretical perspectives in assessing client treatment needs and responses.
- Master’s degree Psych Certified RN, Licensed Clinical Social Worker (LCSW), Licensed Clinical Professional Counselor (LCPC), Licensed Social Worker, or Licensed Professional Counselor (LPC) in the state of Illinois.
