We are seeking an experienced and licensed Mental Health Clinician to provide critical behavioral health services to individuals in a hospital setting.
Key Responsibilities :
- Conduct CalAIM assessments, diagnosis, and documentation.
- Develop treatment plans and maintain detailed progress notes.
- Provide psychoeducation, symptom management, medication management, and independent living skills training.
- Perform therapeutic case management, including resource linkage and support for documentation required for services and benefits.
- Engage and support unhoused individuals, including those not yet linked to services.
- Provide transportation in support of case management activities.
- Participate in housing-related meetings and updates to better support individuals served.
