Title: Case Manager (Going Home)
Type of Position: Full-time employee
Direct Supervisors: Executive Director and Founder/Board Chair
Position Overview:
Grace at the Green Light’s Going Home Case Manager will perform all duties to run the Going Home program reuniting the unhoused population with loved ones. This position will have daily direct contact with unsheltered individuals, providing direct services (food, water, clothing, bus tickets, etc) as well as administrative duties related to program management. Case Manager will also assist in running Meals With Love/Breakfast Program under the direction of Executive Director and other program staff.
Primary Duties:
Meals With Love/Breakfast Program
- Work breakfast up to 5 mornings a week (5:45am-8:00am) in conjunction with Executive Director and other program staff. *Including Saturday mornings*
- Managing volunteers that assist in serving breakfast (welcoming and letting them know what they are doing)
- Making sure clients are welcomed and provided food/water/hygiene items
Going Home Program
- Running Going Home Program that reunites unhoused, stranded individuals with their loved ones via case management and providing bus/train ticket as well to client
- Completing intakes & all required documentation for Going Home program at St Jude Community Center and at other locations as needed throughout the week to insure real-time response
- Complete all intake and reporting documentation per Unity/HUD standards
- Contact family members/conduct due diligence of client
- Purchase bus/train/airline tickets for clients that qualify for programming
- Purchase food for clients
- Meet clients at the bus/train station to see them off
- Input all data from Going Home program into HMIS (Homeless Management Information System)
- Conducting follow up by phone call for clients in our Going Home program and provide support
Case Manager will also assist Executive Director & other program staff as needed to ensure services offered to our unhoused community are consistent and offered in a compassionate manner.
Working with the unhoused community is a demanding, high stress job. Individual must have high energy and be willing to work with individuals and groups in challenging situations which may include substance use disorder and mental health crisis.
Job Type: Full-time
Pay: $45,000.00 per year
