Position Summary
The Client Care Manager is responsible for overseeing the personal care and homemaker services provided by our care team to clients while ensuring satisfaction and compliance. This role includes client case management, caregiver supervision and oversight, training and education and quality assurance to uphold the high standards of Grace Givers Home Care. The Client Care Manager will also serve as an on-call point of contact for urgent client or caregiver needs outside of normal business hours on a rotation with other management staff.
Essential Duties & Responsibilities
Administration and Maintenance of Grace Givers Caregiver Education Program
Administration and Maintenance of Client Care Plans and Assessments
Administration and Maintenance Grace Givers Quality Assurance Program
Participates in On-Call Rotation as Needed
Assures proper documentation of all work-related activities.
Assists with Client Discharge Planning
Participates in Corrective Action Procedures and Performance Improvement Plans
On-Call Responsibilities
Answer and manage phone calls during on-call hours.
Conduct client introductions, handle call-outs, and perform care consultations as needed.
Complete an on-call documentation
Monitor Clock Ins and Clock Outs during On Call Hours
Qualifications & Skills:
Previous experience in caregiving required.
Ability to communicate effectively required.
Previous experience in home care, healthcare, or caregiving management preferred.
Strong communication, organizational, and problem-solving skills.
Ability to give and receive constructive criticism.
Proficiency with technology including tablets, computers, Microsoft Word, Excel, Adobe, and email knowledge, and ability to self-seek learning resources for assistance with basic technology support.
Ability to manage schedules and resolve caregiver or client concerns efficiently.
Proficiency in Wellsky or similar scheduling/documentation software preferred.
Availability to work flexible hours, including on-call responsibilities.
Compassionate, detail-oriented, and dedicated to providing high-quality care.
May fill in for caregivers if needed and must be able to perform all duties of the Grace Givers Care Giver Position Description.
Must be able to lift, bend, stoop, and push or pull up to 50 lbs.
Other Duties as assigned.
