What are the responsibilities and job description for the Community Support Professional position at Grace Harbour?
Grace Harbour, Inc., a CARF accredited Behavioral Health Core Service Provider, is seeking qualified Georgia licensed applicants (LPC, LAPC, LCSW, LMSW, LMFT, LAMFT) to conduct community based counseling in the Coweta County area. Applicants must be willing to provide clinical services in an office setting and/or within the community (i.e., at schools, etc.). Applicants must be flexible, organized, computer literate, and detail oriented. All applicants must submit to random drug screens and pass a Federal Background Check. Grace Harbour, Inc. offers free supervision for licensure, free CE opportunities, and a supportive clinical team that embraces professional and personal growth. Hourly pay to be commensurate with experience and education.

High School diploma - $26 - $28
Bachelors degree - $34 - $36
Job Duties
- Provide in-home, in-community, and in-school skills-based services to assist clients and families in meeting identified goals (example: coping skills education, parenting skills training, etc.)
- Link families to resources in the community, based on needs
- Collaborate with other Grace Harbour staff and community partners to ensure effective treatment, service provision and case collaboration
- Planning in a proactive manner to assist the youth/family in managing or preventing crisis situations
- Attend regular team meetings
- Attend agency trainings, as needed
- Complete progress notes according to internal and external agency guidelines within designated timeframes
- Develop, Review and update Treatment Plans as needed
- Complete and submit service authorization requests for clients
- Opening and closing Grace Harbour, Inc. facility per requisite need
- Obtaining/adhering to appropriate releases of information in accordance with HIPAA/ CFR 42 guidelines Regular reports, written and/or verbal, to multiple referral agents which may include: probation, parole, judicial agents, attorneys, DFCS, DHR as well as other professional client agents
- May be required to participate in different committees
