Job Summary
This is a PART-TIME position that may grow into Full-Time.
We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will provide comprehensive care to patients of all ages, focusing on health promotion, disease prevention, and management of acute and chronic illnesses. The FNP will work collaboratively with physicians and other healthcare professionals to ensure optimal patient outcomes in a supportive environment.
Duties
- Conduct thorough patient assessments, including health histories and physical examinations.
- Develop and implement individualized care plans based on patient needs and preferences.
- Provide health coaching to patients, guiding them in lifestyle changes and self-management strategies.
- Manage acute care situations effectively, ensuring timely interventions for patient safety.
- Collaborate with multidisciplinary teams in discharge planning for patients transitioning from hospital to home or other care settings.
- Utilize management skills to coordinate patient care effectively within a Level I Trauma Center.
- Provide specialized care for infants and children, including those requiring PICU services.
- Support patients with disabilities by adapting care plans to meet their unique needs.
- Engage in ongoing education and professional development to stay current with best practices in nursing.
Experience
Candidates should possess the following skills and experiences:
- Proven experience in utilization management within a clinical setting.
- Proficiency in health coaching techniques that empower patients.
- Experience in providing care for patients requiring dialysis.
- Familiarity with infant care practices and pediatric assessments.
- Background in acute care settings, particularly within trauma or critical care environments.
- Strong skills in patient assessment and discharge planning processes.
