Job Description
- Coordinate the patient/family care and services including those of the IDT. (interdisciplinary team)
- Appropriately utilizes the skills and resources of contract personnel such as physical therapy when appropriate.
- Develops and updates plan of care with the IDT and others involved in the patients care.
- Provides skilled nursing as determined by the plan of care.
- Educates patient/family and other primary caregivers, in appropriate care techniques.
- Serves as the patient/family advocate by maintaining regular communication with attending physicians, long term care facility staff, HMO case managers, and others as well as .
- May participate in providing bereavement support to families and associated bereavement activities.
- Ensure and document appropriate use of medication, durable medical equipment and supplies
- Participates in team meeting.
- Provide appropriate, coordinated care plans.
- Monitors the recertification/reauthorization process
- Reports necessary information to the Team Leader and other members of the IDT for on-call shift report.
- Responds to on-call issues as clinically appropriate
- Performs on-site evaluations and supervision of Grace HHAs.
Qualifications
- Currently licensed to practice nursing in the state
- A minimum of two years nursing experience in an acute-care hospital nursing in either medical, surgical or oncology unit or other commensurate experience.
