Purpose :
The person in this position will provide direct casework and counseling services to the hospice patient / family as directed by the plan of care. The social worker provides psychosocial assessments and develops a corresponding care plan as coordinated with the Interdisciplinary Team
Job Description :
Provide psychosocial assessments, direct casework, and counseling services to patients and their families.
Participate in the preparation, evaluation, and execution of the interdisciplinary care plan to assure that the aims of continuity and comprehensiveness of care are accomplished.
Work collaboratively as a member of the Interdisciplinary Team(s).
Prepare clinical and progress notes for the clinical record per Hospice, state, and federal guidelines / standards.
Prepare documentation for the clinical record per Hospice, state, and federal guidelines and standards.
Maintain and update knowledge of community resources. Facilitate referral of patients / families to those resources as needed and desired.
Participate in taking on-call responsibilities as necessary including weekends and evenings.
Participate in the Interdisciplinary Team meetings, staff meetings, and other meetings as requested.
Qualifications :
A Master's degree from an accredited educational institution of social work is preferred or another related clinical degree
Must be a licensed social worker
Strong interpersonal and communication skills are required
Experience in health care and clinical social work is preferred
Knowledge of the process of death, dying, and bereavement is required
Understanding of psychosocial factors operative in terminally ill patients and families required
