Founded in 2004, Grace Outreach is a female-focused organization that serves underserved women 18 years and older who seek to improve their academic status by securing their High School Equivalency diploma, pursuing higher education, and / or gaining living-wage employment. Grace Outreach originally started as a high school equivalency education center but has grown into a successful larger organization to include additional services that align with the goal of our clients achieving self-sufficiency.
The Grace Outreach LMSW will play a pivotal role in the success of our programs and the advancement of our students. This position, in partnership with the Student Engagement Coordinator, serves as the cornerstone of student support, ensuring that individuals receive the personalized guidance they need to overcome barriers and stay on track with their educational and career goals. By providing case management, monitoring attendance, verifying compliance with critical documentation, and addressing personal and systemic challenges, the social worker creates a stable foundation for student success. Their expertise in advocacy, resource navigation, and emotional support empowers our students to thrive, making this role indispensable to achieving Grace Outreach’s mission of transforming lives through education and opportunity.
The LMSW will provide direct services to students, collaborate with staff, and contribute to creating a positive and supportive learning environment. The LMSW will work with students to address any challenges that may impact their academic success, social interactions, and overall mental health.
RESPONSIBILITIES (to include but not limited to)
- Develop preventative services to support social / emotional wellness.
- Provide individual counseling to students helping them navigate personal, academic, and career challenges.
- Meet with students individually or in groups to discuss the importance of consistent attendance, encourage accountability, and ensure students understand and comply with the organization’s attendance policy.
- Crisis intervention - address immediate needs or crises, including housing, instability, food insecurity, or mental health concerns, connecting students to appropriate resources.
- Advocate for students within the community, ensuring they have access to essential services.
- Lead group sessions on life skills, stress management, and other topics relevant to students’ development.
- Offer short-term counseling and emotional support and refer students to long-term mental health professionals when necessary.
- Collaborate with the College and Careers team to help students identify career or college paths, apply for vocational training, and prepare for employment.
- Maintain calendar and confidential records of counseling sessions, intervention measures, and referrals with students in Salesforce CRM.
- Respond and report emergencies, provide immediate support, and follow up.
- Collection of quantitative and qualitative data during the intake process to determine the needs of students.
- Regularly track and review students’ attendance records, identify patterns of absenteeism, and conduct follow-ups with students and the Student Engagement Coordinator to address barriers impacting their attendance.
- Respond to student inquiries, maintain email records and phone communication with current, past, and prospective students, and follow up as needed.
- Work closely with staff to develop and implement strategies to support students' academic and emotional growth.
- Participate in community coalitions and / or community groups
- Maintain relationships with local social services, employers, and community organizations to create a network of resources for students.
- Assist in developing and refining programs that address students’ evolving needs and participate in evaluating their impact.
- Maintain accurate records of student interactions and progress, complying with organizational and legal confidentiality requirements.
- Work effectively with Grace Outreach's diverse student population, respecting cultural backgrounds, experiences, and values.
- Responsible for verifying student attendance and ensuring the accuracy and compliance of required documents, including HRA, FIA, and other attendance record documentation.
