What are the responsibilities and job description for the Family Advocate position at Grace's Table?
Work Environment: This role is expected to work a regularly scheduled 20-30 hours/week onsite, with occasional evening events.
Position Summary
The Family Advocate is responsible for five areas related to the care of teen mamas and their kiddos: relationship development; resource navigation; goal setting and attainment; group facilitation/support; and program events. Family Advocates also develop relationships in the community to expand support/growth options for Grace’s Table participants. Family Advocates oversee the volunteer mentors who work with the mamas in their caseload.
Responsibilities
- Conduct program intake meetings
- Regularly meet with mamas on your case load to build relationships and support their goals.
- Develop community resource and goal attainment plans with mamas on your case load.
- Complete routine charting in case management system related to mamas on your case load and/or attend groups.
- Coordinate and facilitate weekly groups as assigned.
- Facilitate Educate and Empower programs in area schools
- Support mentor matches and assist in the transition in and out of the partnership.
- Develop and maintain relationships with local human services, education, and career development organizations.
- Manage the transition process for mamas approaching graduation from GT.
- Collaborate with Grace’s Table team to identify areas of concern/opportunity and solutions.
- Assist in the creation of and the execution of special events.

Qualifications
- Bachelor’s degree or equivalent experience
- Two years of case management or program facilitation is preferred
- Experience working with at-risk populations is preferred
- Must pass rigorous background checks
- Valid driver’s license and current automobile insurance
Physical Demands:
- Physical requirements include a typical office environment including sitting at a computer screen and exerting up to 30 pounds of force occasionally.
- Must be able to access and navigate each department at the organization’s facilities.
- Ability to climb, balance, stoop, kneel, crouch, and reach.
- The job entails standing for periods of time.
