Job Summary
Care Coordinators at Grace Skilled Nursing & Therapy play a vital role in ensuring the highest quality of care for our residents. As a Unit Manager, you will be responsible for overseeing the day-to-day operations of your unit, supervising and directing nurse staff, and collaborating with our interdisciplinary team to develop and implement effective care plans.
About the Role
As a key member of our leadership team, you will be responsible for maintaining and restoring health, addressing concerns from family members, and ensuring that your unit meets federal and state regulatory standards. You will also be responsible for supervising, counseling, and disciplining nurse staff, as well as developing, implementing, and adjusting patient care plans as needed.
Responsibilities and Purpose
- Maintaining and restoring health of residents
- Acting as a main point of contact between residents and their families
- Addressing and preventing concerns from family members
- Assist in developing, implementing and making adjustments to patient care plans as needed
- Supervise, counsel and provide discipline to nurse staff to ensure the highest quality care possible
- Ensure their unit meets federal and state regulatory standards, as well as company policies and procedures
Desired Experience and Education
- Current and unencumbered Oklahoma Registered Nurse (RN) or Licensed Practical Nurse (LPN) License
- Up to date First Aid and CPR certification
- One (1) or more years of experience in a Skilled Nursing Facility or Long Term Care environment preferred
- Knowledgeable of regulatory standards determined by CMS and the State of Oklahoma
