Must have Home Health Experience: admissions/OASIS/Wound Care

Job Summary:
We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. As a Registered Nurse - Home Health, you will provide high-quality healthcare services to patients in the comfort of their own homes. You will collaborate with a multidisciplinary team to develop and implement personalized care plans, ensuring the physical, emotional, and psychological well-being of our patients.

Responsibilities:
- Conduct comprehensive assessments of patients' health status

- Conduct admissions, recertifications and visits
- Develop and implement individualized care plans based on patient needs and goals
- Administer medications and treatments as prescribed by physicians
- Perform physical examinations and provide health coaching to patients and their families
- Monitor patients' progress and make necessary adjustments to care plans
- Collaborate with other healthcare professionals to coordinate patient care
- Educate patients and their families on managing their conditions and promoting overall wellness
- Provide skilled nursing care, including wound care, IV therapy, and medication management
- Assist with activities of daily living, such as bathing, dressing, and feeding
- Document all patient interactions, interventions, and outcomes accurately and timely

Experience:
- Minimum of 2 years of experience as a Registered Nurse in a clinical setting
- Experience in home health or similar community-based nursing preferred

Skills:
- Proficiency in using electronic medical record systems (e.g., Athenahealth) for documentation purposes
- Knowledge of ophthalmology procedures and assessments
- Competency in performing physical examinations and interpreting findings
- Familiarity with ICD-10 coding for accurate diagnosis documentation
- Ability to provide health coaching to patients for disease management and prevention
- Experience in infant care, including feeding, bathing, and developmental assessments
- Knowledge of memory care techniques for patients with cognitive impairments
- Proficiency in aseptic technique for wound care and suture removal
- Strong attention to detail for accurate medical documentation
