POSITION SUMMARY :
The Care Manager for the Children's Outpatient Program is responsible for advocating for and coordinating evidence-based care for individuals and families in need. This role involves collaborating through a process of triage, assessment, planning, and communication to ensure continuity of care. The Care Manager works as part of a multidisciplinary treatment team and reports to the unit or department supervisor.

QUALIFICATIONS :
Bachelor’s degree in a Human Services related discipline and at least 1 year experience in the mental health field. (Child welfare programs require 2 years of experience with at-risk populations and a child welfare certification. ) Ability to read, analyze, and interpret clinical records, procedures, or governmental regulations. Ability to write reports and document clinical activities and effectively communicate verbally with, provide information to, and respond to questions from managers, patients, service providers, managed care entities, family members, and other stakeholders.
PHYSICAL DEMANDS :
Abilities in reaching, bending, talking, fingering, sitting, carrying, standing, grasping, fine hand coordination, ability to hear, ability to read and write, and the ability to remain calm under stress. Ability to reach with hands and arms, climb stairs, balance, stoop, kneel, crouch or crawl. Specific vision abilities include close, distance, color, peripheral, depth perception, and the ability to adjust focus. Must be capable of lifting 50 pounds of dead weight alone. May be required to complete standing presentations.
WORK ENVIRONMENT :
The work involves a fast-paced, collaborative team environment that focuses on critical thinking and decision-making, guided by a program supervisor or manager. The office is climate-controlled, and there is regular interaction with clients. Some programs may require travel within Hillsborough County, either using a personal vehicle or an agency vehicle, to provide in-home client services or conduct onsite clinical programs in various socioeconomic neighborhoods.

Mon-Fri 8 : 00AM- 4 : 30pm
