This job posting is for the overnight shift, 12 midnight to 8 : 30am Sunday-Thursday.POSITION OVERVIEWA Care Manager provides direct care to individuals served with behavioral health needs which includes supporting individuals served with coordination and continuity of evidence-based care for individuals and families in need through a collaborative process of triage, assessment, planning, and communication. - Must have a Bachelor's Degree in Human Services or a related field of study toward Human Behavior / Science.
- Must be able to lift and carry up to 50lbs of dead weight
- Must have excellent interpersonal skills
- Valid Florida Driver's license, reliable personal transportation, and insurance.POSITION RESPONSIBILITIES
- Completion of thorough assessments and coordination of community resources based on patient needs as per program requirements.
- Complete all clinical and administrative documentation based on established timelines as per program requirements.
- Communicate effectively and maintain cooperative relationships with internal and external resources.
- Develop service plans, treatment plans and / or aftercare plans with clear goals and objectives as per program requirements.
- Maintain efficient workflow management and organization as per program requirements.
- Maintain proficient quality of clinical documentation as per program requirements.
- Other duties as assigned or needed as per program requirements.WORK ENVIRONMENT :
- Secure facility that provides inpatient treatment programs for those experiencing a mental health emergency needing 24-hour care in a safe supportive environment.
- This is a 60-bed Adult Crisis inpatient served care facility.
- Office setting within the Crisis center
- Must be capable of lifting 50 pounds of dead weight alone.12 : 00 AM -8 : 30AM
