Position Summary:
We are seeking a compassionate and dedicated LPN/RN to provide high-quality nursing care for individuals living in our group home. The ideal candidate will be responsible for ensuring the health and well-being of residents, managing their medical needs, and collaborating with the care team to create individualized care plans. The LPN/RN will work closely with other healthcare professionals to deliver outstanding support and ensure the home is a safe and nurturing environment.
Key Responsibilities:
- Patient Care:
- Provide nursing assessments, evaluations, and administer prescribed medications and treatments.
- Monitor and document residents’ health conditions, including vital signs, symptoms, and behaviors.
- Assist with activities of daily living (ADLs), including bathing, dressing, grooming, and feeding.
- Provide wound care, catheter care, and other specialized nursing care as needed.
- Care Planning & Coordination:
- Develop and update individualized care plans for each resident, ensuring they meet their unique physical, emotional, and mental health needs.
- Collaborate with doctors, specialists, and caregivers to provide holistic and effective care.
- Maintain accurate and timely medical records, following legal and regulatory guidelines.
- Medications & Treatments:
- Administer prescribed medications and ensure proper documentation of all dosages.
- Educate residents and their families about medication management and side effects.
- Ensure the safe storage and handling of medications.
- Health Education:
- Provide residents and their families with health education and resources to promote independence and improve health outcomes.
- Offer guidance and support for maintaining a healthy lifestyle and managing chronic conditions.
- Emergency Response & Reporting:
- Respond promptly and appropriately to medical emergencies.
- Report any significant changes in health conditions to the appropriate healthcare providers or authorities.
- Collaboration & Teamwork:
- Work closely with other healthcare staff, caregivers, and family members to ensure comprehensive care.
- Participate in team meetings to discuss resident care, goals, and strategies for improvement.
Qualifications:
- Education & Licensing:
- Current and valid LPN or RN license in the state of Maryland.
- CPR and First Aid certification required.
- DDA training certificates.
- Experience:
- At least 2 years of experience in nursing, preferably in a group home or similar setting.
- Experience working with individuals with developmental disabilities, mental health conditions, or elderly populations preferred.
- Skills:
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively both independently and as part of a team.

- Other Requirements:
- Ability to lift and move patients as needed.
- 12 hour shifts.
