LPN/RN

Position Summary:

We are seeking a compassionate and dedicated LPN/RN to provide high-quality nursing care for individuals living in our group home. The ideal candidate will be responsible for ensuring the health and well-being of residents, managing their medical needs, and collaborating with the care team to create individualized care plans. The LPN/RN will work closely with other healthcare professionals to deliver outstanding support and ensure the home is a safe and nurturing environment.

Key Responsibilities:

• Patient Care:
• Provide nursing assessments, evaluations, and administer prescribed medications and treatments.
• Monitor and document residents’ health conditions, including vital signs, symptoms, and behaviors.
• Assist with activities of daily living (ADLs), including bathing, dressing, grooming, and feeding.
• Provide wound care, catheter care, and other specialized nursing care as needed.
• Care Planning & Coordination:
• Develop and update individualized care plans for each resident, ensuring they meet their unique physical, emotional, and mental health needs.
• Collaborate with doctors, specialists, and caregivers to provide holistic and effective care.
• Maintain accurate and timely medical records, following legal and regulatory guidelines.
• Medications & Treatments:
• Administer prescribed medications and ensure proper documentation of all dosages.
• Educate residents and their families about medication management and side effects.
• Ensure the safe storage and handling of medications.
• Health Education:
• Provide residents and their families with health education and resources to promote independence and improve health outcomes.
• Offer guidance and support for maintaining a healthy lifestyle and managing chronic conditions.
• Emergency Response & Reporting:
• Respond promptly and appropriately to medical emergencies.
• Report any significant changes in health conditions to the appropriate healthcare providers or authorities.
• Collaboration & Teamwork:
• Work closely with other healthcare staff, caregivers, and family members to ensure comprehensive care.
• Participate in team meetings to discuss resident care, goals, and strategies for improvement.

Qualifications:

• Education & Licensing:
• Current and valid LPN or RN license in the state of Maryland.
• CPR and First Aid certification required.
• DDA training certificates.
• Experience:
• At least 2 years of experience in nursing, preferably in a group home or similar setting.
• Experience working with individuals with developmental disabilities, mental health conditions, or elderly populations preferred.
• Skills:
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively both independently and as part of a team.
• Compassionate and patient-focused approach to care.
• Other Requirements:
• Ability to lift and move patients as needed.
• 12 hour shifts.

Pay: $35.00 - $40.00 per hour
