JOB SUMMARY:
The Clinical Care Coordinator (C3) proactively identifies and removes barriers that my impede department or system wide patient flow by serving as a consultant and educator with medical staff regarding patient status, bed placement, payor requirements and utilization of resources. The position (Unit C3) will act as the primary liaison between emergency and surgical services, nursing, behavioral health, finance, physicians, utilization review to ensure appropriate bed is assigned, patient care needs are met and treatment plans executed at the appropriate levels. Will work collaboratively with all members of the healthcare team to improve patient throughput and length of stay (LOS) performance of the organization.
JOB QUALIFICATIONS:
Bachelor's Degree in a health related field is required
Three (3) or more years of clinical experience is required
Nurse Case Management experience is preferred
Registered Nurse in the State of Georgia required.
Basic Life Support (BLS) Certification is required.
For Critical Care and ECC units Advance Cardiac Life Support (ACLS) is required.
Certified Case Manager (CCM) or American Case Manager (ACM) Certification is preferred
