JOB SUMMARY:
The Community Health Worker collaboratively works with partners in the Grady Health System and community to provide comprehensive community outreach services and health education to high utilizers of Grady Hospital.
WHAT YOU WILL DO:
· Prepares reports and documentation as needed.
· Provides referrals and support for targeted cases.
· Conducts patient education/health education.
QUALIFICATIONS:
· Bachelor's degree in public health, social services or related field OR 2 years of relevant experience in lieu of education.
· Experience providing health education is preferred.
· Must be proficient in Microsoft Office.
· Must possess a good knowledge of interviewing techniques; of the surrounding community; program policies; and some knowledge of the health care systems.
