- Assesses, plans, and implements rehabilitation services and for speech, language, and/or swallowing disorders.
- Provides evaluation of speech and language abilities through diagnostic testing, establishes goals and estimates prognosis for recovering.
- Maintains and updates professional competency through in-service attendance and similar professional resources.
- Records and reports the client's progress in treatment and any change in patient's condition to the physician.
- Instructs family member(s) and members of the health care team in methods of assisting the patient in improving communication skills.
- Re-evaluates patient's progress and the established rehabilitation program as often as necessary.
- Prepares periodic reports, as required by the Agency, which are sent to the physician and third party payers regarding care being provided.
- Assists in making appropriate referrals to outside audiology speech clinics or other community services as indicated.
- Assists in the development and revision of physician's Plan of Care.
- Provides liaison contact and follow-up with other community agencies.
- Participates in in-service, case conferences, and staff meetings as required.
- Participates in staff and Agency development activities.
- Maintains and updates professional competency through in-service attendance and similar professional resources.
- Maintains adequate clinical and progress notes and submits all required paperwork in a timely manner according to Agency policy.
- All other duties as assigned by supervisor.
- Must be able to use good and sound clinical judgment.
- Must be able to document accurately and appropriately.
- Must follow up as needed and appropriately with doctor's office, clinical and management staff pertaining to any concerns regarding patient care.
- CPR certified.
- Master's degree from a speech and hearing program certified by the American Speech and Hearing Association to practice in the state in which you work.
- Must hold, in good standing, a license/registration as a Speech Therapist, issued by the state in which you work.
- A minimum of one (1) full year of supervised post-certification experience in an institutional setting.
- Home health care experience desirable.
