Description

ORGANIZATIONAL DESCRIPTION
The position provides pharmacy services, inventory management and other pharmacy duties in accordance with state or federal Pharmacy laws and in close collaboration with the Pharmacist. The position requires availability or delegated availability to respond after working hours. Takes rotational call.
INTERACTION
Primarily Interacts with: administration, physicians, employees, clinical and non-clinical departments, patients, and visitors.
ESSENTIAL FUNCTIONS
  • Administer medications to patients as prescribed by the physician and assesses the effects of the medication.
  • Demonstrate knowledge of dosage, route, action, side effects and contraindications of medications. Utilizes resources as necessary.
  • Administer medications and narcotics following hospital policies and procedures.
  • Identify drug interactions.
  • Knowledge of antidotes/treatment for medication overdose.
  • Knowledge of antidotes/treatment for medication infiltrations.
  • Calculate all dosages accurately.
  • Document appropriately.
  • Able to set up and operate equipment on unit. Report all damaged equipment to department/person.
  • Keep work area clean and neat. Replace, remove, and return equipment and supplies as appropriate.
  • Follow procedures for body substance isolation system. Seek assistance as needed.
  • Review medication orders and dispenses all necessary medications within 30 minutes of receiving the order.
  • Accurately enter orders into the pharmacy computer system at a volume consistent with workload requirements, including all medication orders received within 30 minutes of closing.
  • Fill and replace medications and IV solutions to the automated dispensing machines on a daily basis with extreme accuracy. Fill and replace medications to all other ancillary departments (ie: Surgery, OP) as required at least on a weekly basis
  • Package and prepare medications in unit-dose containers and as required for ER dispensing. Replace ER dispensing medications several times a week.
  • Perform and complete all daily pharmacy activities related to medication delivery.
  • Reorder all medications as needed from the selected pharmacy vendor utilizing group-purchasing contracts.
  • Reorder the pharmacy with IV solutions and supplies as ordered from Material Management.
  • Order over-the counter (OTC) medications for employees (with assistance from the employee as to specific needs). Complete all necessary paperwork for sales of these medications.
  • Clarify every unclear medication order with the physician or nurse. Notify the pharmacist of any problem.
  • Perform medication delivery functions with a minimum of errors, no more than a few per year.
  • Inspect all medications purchased by pharmacy. Check against the invoice. Maintain purchasing records and accurately records all information necessary for narcotics and controlled substances.
  • Accurately replace to all ancillary areas narcotics and controlled medication without error. Immediately notify the pharmacist of any problem or discrepancy regarding controlled substances.
  • Conform to pharmacy policies and procedures, objectives, quality assurance program (including reporting of medication errors), safety, environmental and infection control standards, and disaster plan.
  • Learn and adhere to the rules and regulations of the Texas State Board of Pharmacy concerning Class C License; including drug recall; out-patient dispensing of medications; monthly medications inspection in ancillary areas drug interactions; etc.
  • Conform to hospital policy on Drug Formulary by use of formulary medications and notifying pharmacist for approval of use of a non-formulary drug.
  • Set priorities and assures the completion of care for all assigned patients.
  • Give pertinent, accurate, concise organized reports and documents on record accordingly and inputs or retrieves information from EMR systems.
  • Develop, interpret, implement, evaluate, and document a plan of care for each patient using the nursing process.
  • Recognize and report significant changes in patient’s condition to appropriate person. (i.e. charge nurse, physician)
  • Assess patient’s response to his illness and hospital environment.
  • Initiate written plan of care for each patient within 24 hours of admission
  • Review nursing plan of care daily and as patient status changes. Update daily by making entry on care plan and nursing notes.
  • Include patient, family and other members of health care team, as appropriate in planning care.
  • Utilization of appropriate interpersonal skills when collaborating with interdisciplinary team members.
  • Implementation of the medical plan of care.
  • Develop care plan that involves other disciplines of patient assessment, interventions and evaluations of that care.
  • Manage self, patient and other employees in any emergency situation.
  • Provide exemplary customer service.
  • Maintain confidentiality and compliance with HIPAA.
  • Cooperate with other personnel to achieve departmental objectives and maintain good employee relations, inter-departmental objectives and infection control policies.
  • Cordially demonstrate timeliness and professionalism in appearance, communications, and actions during all interactions and with all individuals and entities.
  • Solve practical problems, dealing with a variety of issues, and interpret a variety of instructions furnished in written and oral form.
  • Attend meetings as required.
  • Perform other duties as requested or assigned.

Requirements

ABILITY TO USE EQUIPMENT, TOOLS, MATERIALS, MACHINES: Computer, telephone, facsimile machine, copier, scanner, printer, calculator and other standard office equipment. Standard Pharmacy equipment.
EDUCATION/EXPERIENCE/SKILLS/ABILITIES
Education:
  • High school diploma or equivalent required.
  • Valid licensed vocational nursing license in Texas
  • Pharmacy technician certification
  • BLS certification

Experience:
  • One year nursing experience preferred.
  • Medical office or healthcare experience preferred.
