What are the responsibilities and job description for the Community Housing Specialist position at Grand Mental Health?
§ Provide a variety of office and field activities to coordinate, manage and monitor a community housing or placement program for individuals, performing direct client services and compiling all related documentation.
§ Provide varying types of case management in various settings to individuals, families, and groups.
§ Provide crisis intervention services as assigned.
§ Studies and evaluates the housing needs of clients and matches those needs with the current available housing in the community.
§ Researches and develops housing library or list of open/available properties to assist all candidates in obtaining permanent housing.
§ Assist clients in securing financial move-in assistance such as security deposits, rental assistance, and eviction prevention systems, when available.
§ Works to build relationships with all housing options throughout the community, such as property managers, private landlords and real estate companies.
§ Possess a working knowledge of all homeless shelters in the area, community housing programs including low-income options, housing options for sex offenders or others with criminal backgrounds.
§ Completes all agency documentation of services in a thorough, accurate and timely manner.
§ Assists clients to adjust and adapt themselves to their environment, care, and treatment.
