Qualifications
- Current Idaho State Dental License
- Current D.E.A. Certificate
- Insurability (malpractice)
- Current Cardiopulmonary Resuscitation Certificate (CPR)
- ACLS or Advanced Life Support within 12 months of employment, if appropriate
- Must be eligible to participate with CMS programs
