BASIC FUNCTION:
The Care Center Nurse Practitioner provides high-level medical care for residents who need ongoing or post-acute care. The NP works closely with physicians, nursing staff, and other healthcare providers to assess, diagnose, treat, and manage the health conditions of residents. Their role includes coordinating care, developing treatment plans, and promoting patient health and well-being.
SHIFT INFORMATION:
- Full-Time or Part-Time available, negotiable
- Full-Time will be blended with Day-Of Clinical care shifts.
- Part-time will be three days per week.
- New Care Center facility built in 2021
SALARY INFORMATION:
- $60.00 Dependent on experience may vary outside this range
- Bonus (If Eligible)

RESPONSIBILITIES:
- Assesses needs, incorporates plans and provides direct patient care for residents at our long term care facility.
- Examines residents, performs comprehensive physical examinations and compiles and documents residents medical data including health history, progress notes and results of examinations and treatment plans.
- Takes actions to ensure proper illness and injury care, disease prevention, diagnosis, treatment, and recovery.
- Works in collaboration with Physicians and other interdisciplinary members of care teams.
- Orders, interprets and records results of a variety of clinical tests and reports results to other Providers and departments as needed.
- Conducts resident education.
- Prescribes medications, order diagnostic tests, and makes recommendations for other therapeutic forms of treatment.
- Participates in quality assurance planning, initiatives and improvement projects.
- Attends various meetings and serves on committees as assigned.
- Relies on extensive experience and judgment to plan and accomplish goals.
- Functions under the direction of assigned supervising Physician.
- Performs a variety of tasks and other duties as assigned.
QUALIFICATIONS:
- Preferred specialization in geriatrics, to understand and treat the often complex physical and mental health needs of residents.
- Requires active Colorado licensure as a Certified Registered Nurse Practitioner in area of specialty based on licensure and Colorado authority for advanced practice.
- ACLS and PALS certifications may be required within 6 months of hire depending on admitting privileges.
- Required certifications and licensures must be maintained in current status throughout employment.
- 3 to 5 years working experience in a clinical setting in a similar role preferred.
