Job Summary
The Grand Traverse Band of Ottawa and Chippewa Indians is seeking a highly skilled Community Health Representative to join our team. As a key member of our community healthcare team, you will play a vital role in delivering high-quality, confidential services to our tribal members.
Key Responsibilities
- Provide health promotion and education services to individuals and families in the community.
- Assist with maintaining patient rights to confidentiality of health information.
- Work collaboratively with healthcare professionals and other resource agencies to assist clients' medical and social needs.
- Establish a cooperative network with healthcare professionals and other resource agencies to assist clients' medical and social needs.
- Provide home visits and follow-up care services to tribal members.
- Coordinate work schedule and client appointments with the Satellite Office Receptionist.

- Provide health education and promotion services to tribal members.
- Assist with maintaining patient records and documentation.
Requirements
- High school diploma or equivalent.
- Current Michigan Certified Nurse Aide certification or completion of CNA training within 90 days of hire date.
- Basic knowledge of medications and medical terminology.
- Valid unrestricted driver's license and insurability by the GTB insurance carrier.
Working Conditions
The Community Health Representative will work in a variety of settings, including homes and the satellite office. The position requires frequent driving to client homes and medical appointments.
