Responsibilities
The Hospital Chaplain, in accordance with the plan of care approved by the attending physician when applicable, the Hospital Chaplain assists the Interdisciplinary Group (IDG) in assessing and meeting the spiritual needs of the patient/patient’s family and/or caregivers. The essential functions of the Hospital Chaplain are:
Essential Functions:
- Provide spiritual support for all patients/families/caregivers as requested.
- Provide pastoral care that respects diversity and differences including, but not limited to culture, gender, sexual orientation, and spiritual/religious practices.
- Assesses patient/family/caregivers spiritual needs through an initial visit or telephone contact/assessment.
- Provide Spiritual/Pastoral Care to patients/families/caregivers experiencing loss and grief.
- Facilitates and attends trauma training to assure patient and family support.
- Provide effective spiritual/emotional support for hospital staff and volunteers as needed
- Communicate with patient’s community spiritual support person as indicated/requested. Acts as a liaison to the spiritual community as indicated.
- All contacts related to a particular patient are documented - in the patient’s medical record.
- Changes in the POC or patient’s condition are communicated directly to the IDG via phone, secure text , EMR and or during IDG meetings.
Schedule: Per Diem, On call between 4:00pm - 8:00am, some weekends.
Qualifications
Education and Experience:
High School Diploma/GED required Bachelor's Degree preferred Completed at least one unit of Clinical Pastoral Education (CPE) or equivilent, required
