Position Overview :
We are looking for a dedicated Physical Therapist Assistant to join our home health team. In this role, you will assist in the delivery of high-quality physical therapy services to patients in their homes, helping them regain independence, mobility, and quality of life. You’ll work alongside a skilled team of therapists, while having the autonomy to manage your own caseload and schedule.
Key Responsibilities :
Assist physical therapists in implementing patient treatment plans and documenting progress.
Provide hands-on physical therapy treatments and exercises to help patients improve strength, mobility, and function.
Educate patients and caregivers on home exercise programs, safety techniques, and recovery goals.
Collaborate with physical therapists, nurses, and other healthcare providers to ensure comprehensive patient care.
Ensure accurate and timely documentation of patient records and progress notes.
Maintain communication with patients’ families and caregivers to enhance care and ensure compliance.

Qualifications :
Licensed Physical Therapist Assistant (PTA) in California
Home health experience is a plus.
Ability to work independently and manage time effectively.
Strong interpersonal and communication skills.
Valid driver’s license and reliable transportation.
