GrandCare is seeking a compassionate, skilled, and motivated Physical Therapist to join our home health care team. In this role, you will provide one-on-one therapy to patients in their homes, develop personalized treatment plans, and work closely with our multidisciplinary team to ensure the highest quality of care in the following Los Angeles cities and surrounding areas:
Carson, El Segundo, Gardena, Hawthorne, Hermosa Beach, Wilmington, Manhattan Beach, Redondo Beach, Rolling Hills, Inglewood, Lawndale, Harbor City, San Pedro, Palos Verdes
Key Responsibilities:
Conduct comprehensive physical therapy assessments and develop individualized treatment plans.
Provide hands-on therapy and education to patients to improve mobility, strength, and independence.
Collaborate with physicians, caregivers, and other healthcare professionals to ensure optimal patient outcomes.
Monitor patient progress and adjust treatment plans as necessary.
Educate patients and families about therapeutic techniques, home exercises, and safety practices.
Maintain accurate and up-to-date patient documentation and treatment records.

Qualifications:
Licensed Physical Therapist (PT) in California
Home health experience preferred but not required.
Strong interpersonal and communication skills.
Strong tech skills (comfort with various EMRs and using tablet for documentation)
Ability to work independently and manage a flexible schedule.
Valid driver’s license and reliable transportation.
