GRANDVUE MEDICAL CARE FACILITY has an excellent leadership opportunity for an RN/BSN to join our state-of-the-art facility as a Care Coordinator. Successful candidate will possess strong clinical and assessment skills, experience in coordination of Resident care, strong leadership, communication and team building skills, be a positive, dynamic, evidenced based nurse with a passion for improving the lives of the residents. Long term care experience, dementia experience, MDS experience and certification is preferred, but not required. WE WILL TRAIN THE RIGHT CANDIDATE!
