Grandvue Medical Care Facility is seeking a dynamic and experienced Social Services and Admissions Director to oversee the admissions process and lead our Social Services team. This key leadership role is responsible for ensuring regulatory compliance, managing team operations, and providing exceptional support to residents and their families. Key Responsibilities:
- Lead and supervise the Social Services and Admissions team, fostering development, engagement, and alignment with the facility’s mission.
- Oversee the admission process, ensuring timely, accurate processing of referrals and admissions, and managing bed availability.
- Collaborate with healthcare providers, social service organizations, and families to ensure comprehensive care.
- Advocate for residents and provide support during the transition into the facility.
- Ensure compliance with all regulatory requirements and manage accurate documentation for resident care.
- Prepare reports and manage the department’s budget, staffing, and scheduling.
Qualifications:
- Unencumbered RN license or a Bachelor’s or Master’s Degree in Social Work, with state licensure in Michigan.
- Minimum of 2 years of experience in healthcare, with knowledge of MDS, long-term care regulations, and admission/discharge planning.
- Strong leadership, communication, and organizational skills.
- Experience in problem-solving, process improvement and conflict resolution.
