Position Summary:
Looking for an RN or LPN to conduct and coordinate the development and completion of the resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS), Care Are Assessments (CAA's) and care plan in accordance with state and Federal requirements. As the MDS Nurse you participate as a part of the facility interdisciplinary team (IDT) in the systems and process to manage patient receiving skilled services as assigned.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans in collaboration with the healthcare team.
- Ensure accurate documentation of patient medical records and adhere to HIPAA regulations.
- Monitor patient progress and adjust care plans as necessary, providing education and support to patients and families.
- Utilize EHR systems for efficient data management and reporting.
- Participate in discharge planning and coordinate with other healthcare providers to ensure continuity of care.
Qualifications
- Valid LPN license
- Proficiency in EHR systems and medical documentation
- Strong understanding of DRG, CPT coding, and ICD-9
- Experience in patient care, including care plans and discharge planning
- Knowledge of HIPAA regulations and medical terminology
- Familiarity with managed care and critical care environments
- Experience in nursing home settings or acute care facilities
Job Type: Part-time
Pay: $35.00 - $40.00 per hour
