Overview:
The Clinical Case Manager will assist drug and alcohol counselors with Clinical responsibilities. These duties include and are not limited to intake, initial assessment, initial treatment planning, administrative tasks/projects, discharge summaries, individual assessments with clients and consultation with other professionals in regard to client treatment and services. The Clinical Case Manager will work effectively and cooperatively with the Clinical staff to support clients to abstain from substances and live happy and productive lives free of their addition.
Knowledge:
- Must possess a Bachelor’s degree. May be working towards a Master’s degree or certification
- Thorough knowledge of causes and treatments for addiction and are trained to make decisions based on ethical and professional standards
Skills:
- Able to work collaboratively with multiple health and criminal justice professionals using tact, diplomacy, and discipline
- Provide individual psychotherapy sessions for program participant in the event a clinician is unable to
- Versatility, flexibility, and willingness to work within constantly changing priorities with enthusiasm
Abilities:
- Complete biopsychosocial for admitting clients within 24 hours of admission
- Create and complete initial master treatment plan within 48 hours of client’s admission to facility
- Obtain client signature for master treatment plan within 24 hours of treatment plan creation
- Assist with inter-agency records transfer to Clinical Supervisors of each designated program and adhere to the procedure in excellence
- Assist with documentation of 50 Day Follow Up phone calls for clients who have discharged over 60 days from current calendar day.
- Assist with administration tasks and/or projects including but not limited to: scheduling of staff, policy writing, documentation for efficiency within the company
- Complete documentation of individual, group progress notes within 24 hours of service
- Complete documentation of discharge notes of all residents with 24 hours of discharge
- Collaborate with Clinical Assistants to complete group notes and concurrent reviews in a timely, efficient, and proficient fashion in excellence
- Ability to effectively communicate with people at all levels and from various backgrounds
Other Characteristics:
- Provides daily communication/collaboration with treatment team of residents entering treatment
- Meets with Supervisor on a regular schedule for continued education and supervision of job responsibilities
- Adheres to all HIPAA guidelines while employed at facility
- Participates in continuing education to continually improve skills and abilities and stay abreast of current technologies/practices
