Overview:
The Recovery Case Manager (RCM) will be expected to facilitate the client’s passage through the 12-Steps. Ideally, this will involve steps 1 thru 8. They will also assist clients with inventory instructions in step 4. RCM’s will also meet with their clients regularly on a one-on-one basis. The RCM is the primary contact for a client's family, loved ones, and referral sources. They are responsible for the planning and implementation of client’s aftercare. RCM’s also serve to assist clients with their outside needs (e.g. court hearings, application for unemployment benefits, FMLA).
Knowledge:
- High school diploma or equivalency required
- Extensive experience with Mental Health and Substance Use Disorder industry
- At least 1 year in recovery
- Sponsorship experience
Skills:
- Time management skills and the ability to work on multiple projects simultaneously
- Model recovery with integrity in word and deed
- Coordinate the reading of 5th steps
- Creation of an aftercare plan that realistically meets the needs of the client
- Vetting of client visitors and phone calls
Abilities:
- Help hold clients accountable for their actions in the community and the completion of their work
- Provide the team with progress reports within 24 hours of meeting with them
- Facilitate and provide timely documentation for the following groups: Goal Setting, Recovery Process, and Therapeutic Writing
- Monitor compliance on client contracts, phone policy, “buddy” relations. Ensure that all pertinent information is notated on RSS office board at shift change
- Ability to work in a fast-paced, dynamic environment
- Ability to communicate effectively in written, electronic, and oral form
- Ability to make sound decisions and/or find alternative solutions
Other Characteristics:
- Make weekly call to client’s family (more communication may be required). This communication will be logged in the EMR. Phone calls will address client progress, behavior, contracts, growth opportunities, programming, and aftercare. RCM must be prepared to discuss enabling, codependency, and/or Al-Anon. RCM is responsible for scheduling all calls and face-to-face meetings with family members and/or loved ones
- Family calls are to be within the first 24 hours of admission
- Referral calls are to be made within the first 24 hours of admission. Calls to referral sources are to be made weekly
- Meet with clients at least once a week to monitor the recovery process
- Responsible for sending out pending and scheduled census cues to the team and/or GRC receiving centers
- Completion of conversion reports on a weekly basis with all pertinent information
- Uploading/delegating current ROIs and aftercare plans into EMR
- Client engagement is not limited individual case load
- Introductions are to be made with new clients on caseload within 24 hours of admission; interaction must be logged in the EMR
