Current opening: Full Time position, 4 days per week alternating M-Th, T-F, Holiday expectation. (30 hours per week). This position will require at a minimum the first 90 days of training to be in-house with opportunity to move to hybrid role upon successful completion of orientation and training.
Reporting to the Senior Director of Hospice, the Clinical Operations Resource Nurse is responsible to provide support for home care and hospice patients through telephone triage. This position is instrumental in assessing and prioritizing patient care needs. This role is essential in providing interdepartmental collaboration and support. The Clinical Operations Resource Nurse works collaboratively with home care and hospice leadership and clinical teams.
Essential Functions:
- Triage calls by providing skilled assessment, problem solving and education/instruction
- Supports scheduling with determining priority of open visits
- Documents calls received/initiated in the patient's medical record
- Assists with chart review and monitors quality assurance
- Reviews/enters orders received that impact the patient's plan of care and makes appropriate changes
- Supports with supply approval as needed
Other job functions:
- Acts as a clinical resource to the Agency
- Demonstrates flexibility in the role by accepting other duties as assigned for which qualified

Requirements
- Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC)
- Candidate must be a Registered Nurse
- One-two years of home health or hospice experience
- One-two years of telephone triage experience preferred
