General Description
The Case Manager is responsible for overseeing a case load of customers to provide program assessment, employment focused training, financial literacy, education/career pathways coaching and resource referral for barriers to education, training, and employment. The Case Manager will develop strong relationships with community partners to provide resources for barrier removal to achieve successful customer training. Our work environment is fast-paced, high-volume and performance-based. As such, Case Managers are expected to meet monthly individual and team goals servicing a caseload of 50 to 60 customers. Monthly Commission available to Case Managers who meet and exceed monthly benchmark requirements. The ideal candidate will demonstrate a positive attitude, be results driven and adaptive to change.
Responsibilities
- Excellent multitasking, time management and organizational skills
- Counsel’s individuals to help them understand and overcome personal, social, or behavioral problems affecting their educational, vocational, financial and career goals
- Assists customers in identifying in-demand occupations and career ladders for education plan
- Assists customers in developing a structured education/training strategy
- Facilitates customer orientation
- Provides Motivational Coaching and initial intake assessments
- Monitors customer progress towards education goals
- Processes customer and documents in compliance with funder requirements
- Collects and records customer data and interactions in company and funder data base
- Maintains electronic case files for each customer on caseload, updating customer documentation within the file at least once per week
- Prepares weekly and monthly reports related to assigned caseload using MS Excel
- Maintains weekly outreach contact with all customers to ensure meaningful engagement in program services
- Fosters positive working relationships with all customers and staff
- Performs other duties as assigned
Qualifications
- Commitment to driving extraordinary customer experiences, with demonstrated ability to drive improved outcomes for customers and the business
- Bachelor’s Degree in social worker, business administration, psychology or related field 2 - 5 years prior experience with case management, social work or related work.
- Strong administrative experience with attention to detail, time management, and extreme organization
- Strong verbal, written, and nonverbal business communication skills with a compassionate and professional tone
- Demonstrated success managing a large volume of electronic data, hard copy documentation and ability to organize filing system weekly
- Ability to manage and secure confidential information at all times
- Ability to organize, document, and distribute company monies with absolute precision
- Successful experience working with the public in a positive and professional manner at all times
- Ability to be empathetic while setting measurable targets for customers
- Desire to work with a diverse population of customers with varying needs and high barriers to employment
- Ability to identify resources and connect them to clients as necessary
- Proven record of achieving and exceeding performance benchmarks

- Proficient at using Microsoft Office and database programs
