We are seeking a highly skilled LPN / LVN to join our team of long-term care specialists. As a travel nurse with GrapeTree Medical Staffing LTC, you will have the opportunity to work in various settings, including nursing homes, assisted living facilities, skilled nursing facilities, and memory care units.
About the Job
This is a 13-week contract position with 36 hours per week. The shift schedule is 12 hours, days. You will be working as a long-term care specialist, providing high-quality patient care and support to residents in these facilities.
Responsibilities:
- Provide direct patient care and support
- Conduct patient assessments and develop individualized care plans
- Collaborate with interdisciplinary teams to ensure seamless patient care
- Document patient progress and outcomes
Requirements:
- Current LPN / LVN licensure
- Minimum 1 year of experience in long-term care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
